• Please register through our patient portal, by clicking HERE .
• If you decide not to pre- register, you can still print out our New Patient Packet to assist you in preparing for your office visit.
• Please be ready to produce a urine specimen in the office.
• Your Picture ID or Drivers License
• Your insurance cards and referral forms required by your insurance company
• A list of ALL medications you are currently taking
• Pertinent medical records for the reason of your visit (This includes films from recent X-rays, imaging studies and recent lab work done.)
